Physician Services by Defined Scope of Work

HMI can provide the services described below on an as needed basis:

• Medical Records Review includes analyzing all support documentation for verifying coding and billing (e.g., E/M, CPT procedure codes, and ICD-9-CM diagnosis) and to ensure compliance with CMS rules and regulations. (Minimum review of 200 medical records.)

• Superbill and Charge Item Master includes recommendations for additions, modifications and deletions for updating ICD-9-CM and CPT-4 code assignments and descriptions for maintaining appropriate reimbursement.

• Policy and Compliance includes a concurrent analysis of a clinic policy for documenting in the patient medical record and billing process, as well an assessment of the functions of office personnel.

• Education includes one-on-one in-servicing and training with the physicians and office personnel.

• Coding Assistance includes assistance as needed in the process for coding patient accounts (minimum of 1 day on-site).
